A Case of Recurrent Bacterial Meningitis with Posttraumitic CSF Otorrhea

Abstract

|
A 9-year-old with repeated (four times in 2 years) pneumococcal meningitis after head trauma by traffic accident is reported. RI cisternography demonstrated an abnormal accumulation of radioisotope in the left ear cavity, suggesting concealed CSF
fistulae (otorrehea). The origin of CSF fistulae (bony dehiscence of tegmen mastoideum and dural laceration) had been detected by simple mastoidotomy, The dura defect was successfully repaired with the initial operation used by perichondrium and
temporal fascia. In order to prevent disastrous complications of repeated purulent meningitis, a detailed study to disclose CSF fistulae should be done in cases of posttraumatic meningitis.
